Red flags: MFA not enforced on all external access points. Patch cycle longer than 30 days for critical vulnerabilities. No centralized visibility into endpoint protection status. User access reviews not conducted at least annually.
3. Detection and Monitoring
Prevention is never perfect. Detection capabilities determine how quickly you discover that something has gone wrong — and faster detection directly reduces the impact of a breach.
What to evaluate:
- Is there centralized log collection from firewalls, servers, endpoints, cloud services, and critical applications?

Red flags: Logs collected but not monitored. No 24/7 monitoring coverage — attackers do not keep business hours. Alert fatigue causing legitimate alerts to be ignored. No correlation of events across different data sources.
4. Response and Recovery
When an incident occurs, the speed and effectiveness of your response determines whether it is a minor disruption or a catastrophic event. Response capabilities must be documented, tested, and practiced.
What to evaluate:
- Is there a documented incident response plan with clear roles, responsibilities, escalation paths, and communication protocols?
- Has the plan been tested through tabletop exercises or simulations within the past year?
- Are backup systems tested regularly with full restore verification — not just backup completion confirmation?
- What are the documented recovery time objectives (RTO) and recovery point objectives (RPO) for critical systems, and are they achievable?
- Are backups stored in a way that protects them from ransomware — isolated, immutable, or air-gapped?
- Is there a relationship with an incident response retainer firm, legal counsel experienced in breach response, and relevant law enforcement contacts?
Red flags: Incident response plan that has never been tested. Backups that have not been restored and verified in over six months. No offsite or immutable backup copies. Recovery time objectives that are aspirational rather than tested.
5. Human Factors
Technology cannot compensate for untrained or unaware employees. Human factors are consistently the most exploited attack vector, and assessing this domain requires looking beyond whether annual training was completed.
What to evaluate:
- Is security awareness training conducted regularly — not just during onboarding but on an ongoing basis?
- Are phishing simulations conducted, and are results tracked over time to measure improvement?
- How does the organization handle employees who repeatedly fail phishing simulations? Is there a structured remediation process?
- Is there a clear, easy process for employees to report suspicious activity without fear of blame?
- Do employees in high-risk roles (finance, HR, executive assistants) receive targeted training for the specific threats they face?
- Is there a security champion program or other mechanism for embedding security awareness into business teams?
Red flags: Training conducted once a year with no measurement of effectiveness. No phishing simulations. No easy reporting mechanism for suspicious emails or activity. Culture of blame around security mistakes that discourages reporting.
How to Score and Prioritize Your Findings
Once you have evaluated all five domains, you need a framework for making sense of the findings and deciding where to act first.
Use a Maturity Model
A maturity model provides a structured way to rate each domain on a scale. A common approach:
- Level 1 — Initial. Controls are ad hoc or nonexistent. Security depends on individual effort rather than organizational process.
- Level 2 — Developing. Basic controls are in place but inconsistently applied. Policies exist but are not regularly enforced or updated.
- Level 3 — Defined. Controls are documented, standardized, and consistently applied across the organization. There is a formal security program with executive oversight.

It is tempting to address the easiest findings first. Resist this. Prioritize based on the intersection of likelihood and business impact. Group your findings into tiers:
